Athletic Packet (includes forms below) Packet Info & Check Off Form Emergency Card - English & Spanish Sports Physical Release Form Code of Conduct - 1 page English & Spanish Concussion Information Sheet HMB Cougar Code of Conduct All students must maintain a 2.0 GP	Volunteer Driver Form. All forms must be pre-approved by the principal. Submit your form with a copy of your drivers license and insurance verification to the high school office. Forma Para Conductores Voluntarios
